Residential Qualified Intellectual Disabilities Professional (QIDP) SALARIED EXEMPT POSITION
Summary:
The QIDP'S primary responsibility is to be a liaison and advocate for consumers within the case management programs (CILA, INTERMITTENT CILA program).
REPORTS TO:
Lead CILA QIDP MAJOR RESPONSIBILITIES TO INCLUDE BUT NOT LIMITED TO:
Completing intake procedures and conducting client needs assessments for consumers entering the CILA Program.
Documenting upon an individual's entry into the CILA those services being provided to the individual until an individual integrated service plan is developed.
Explaining the rights enumerated in Section 115.
250 and documenting in the individual's record that this has been done.
Ensuring that each individual receives initial assessments (i.
e.
ICAP and SLOF Assessment) upon entry into the CILA and reassessments (i.
e.
ICAP and SLOF Assessment) annually that shall be documented in the individual's record and the results explained to the guardian.
Ensuring that each individual has required evaluations (psychological and psycho-social) in accordance with DHS guidelines.
Ensuring that each individual receives all benefits (e.
g.
SS, LINK, Medicaid, Housing assistance, etc.
) they are eligible and that all benefits remain current.
Attending the Community Support Team (CST) and Interdisciplinary Team (IDT), ensuring that services are provided through the CST and IDT as required by 59 IL Admin.

230, and providing a written record of CST/IDT meetings.
Developing individual service plans and behavior plans that are based on assessment results, reflect individual/guardian's preference, identify services/supports to be provided and by whom, and state goals and objectives that are measurable, have timeframes for completion, and have a person assigned responsibility (as per Rule 115).
Reviewing and ensuring the service plans identify the CILA chosen with the individual/guardian's participation, indicate the type and the amount of supervision provided to the individual, address goals of independence in daily living, economic self-sufficiency and community integration, include the names and titles of all staff and other persons contributing to the plan, and be signed by the QIDP, the Pass agent, and the individual/guardian.
Monitoring each individual's status in relation to the service plan, and identifying and addressing gaps in the service provision.
Reviewing and documenting at least monthly, that services are being implemented, that services identified continue to meet the needs of each individual, and services need to be changed/modified based on recommendations.
Reviewing and assembling charts to assure that information specified by the service plans is included in each individual's chart.
Training and supervising Habilitation Technicians completing assessments and on implementing/carrying out the goals/objectives for individual service plans.
Observing and documenting at least quarterly each individual's participation at the day program.
Coordinate with the Nurse Trainer in order to ensure that medical appointments are scheduled, ensuring proper documentation is taken to the medical appointments, and ensuring that any changes and recommendations are documented in the consumer's chart and reported to the nurse.
Coordinate (in written format) with the Lead DSP and Nurse Trainer to ensure that transportation is provided to appointments and other community activities.
Ensuring and assessing the status (strengths/needs, level of functioning, disability, etc.
) of new and current consumers so that a determination or modification of habilitation/treatment needs can be made.
Shares rotating on call duties with the Lead DSP and other QIDPs.
Schedules/completes Emergency Drills for CILA homes and Intermittent CILA homes.
Responsible for Human Rights and Behavior Management Committees and meetings (Quarterly and as necessary) in accordance with DHS requirements.
Responsible for participating in all agency fundraising events.
Attend agency meetings and trainings as needed.
Complete miscellaneous assignments (including being assigned to other programs) as needed to assist in the fulfillment of agency objectives.
Qualifications:
Bachelor's Degree and one year of experience working directly with individuals with developmental disabilities and be eligible for QIDP certification through DHS.
