Modern office setting Relaxed atmosphere Duties - Conduct patient assessments to determine healthcare needs and develop individualized care plans - Coordinate and monitor patient care, ensuring timely and appropriate services are provided - Collaborate with healthcare providers, including physicians, specialists, and ancillary staff, to ensure continuity of care - Utilize medical terminology and knowledge of physiology to accurately document patient information and maintain medical records - Provide patient education on disease management Experience - Strong knowledge of medical terminology, patient assessment, and diagnostic evaluation - Experience in case management or care coordination preferred - Familiarity with primary care principles and utilization management processes - Understanding of HIPAA regulations and ability to maintain patient confidentiality Skills:
